Each sheet includes a preprinted form with prompts for message recipient, date, time, caller, caller's company, telephone number, cell or fax, and message taker's name. Outlined checkboxes allow marking message type such as Urgent, Telephoned, Came to see you, Returned your call, Please call, Will call again, and Wants to see you.
