The purchase order book provides ample space for listing up to 12 items in a carbonless three-part format. The triplicate format includes originals in white, duplicates in canary, and triplicates in pink. Each form includes designated areas for recipient's name, address, shipping address, date, date required, shipping method, terms of shipment, and the name of the purchasing agent. The 12-item list allows for the entry of quantity, product description, price, and unit. A wraparound back cover prevents "write through" for clear documentation. Each detached form measures 5-9/16" x 7-15/16". The book contains 50 sets, with larger quantity quotes available upon request.
